29 CFR 1926.451(b)(8):     At all points of a scaffold where the platform changes direction, such as turning a corner, anyplatform that rests on a bearer at an angle other than a right angle were not laid first, and platforms which rest at right angles over the same bearer was not laid second, on top of the first platform. This violation was most rescently observed:      On or about 13 March 2012 at jobsite located at 2408 Mac Davis, Lubbock TX, were more than one section of the the scaffold, platform sections were not laid properlly on direction change. Exposing employees to fall hazard from shift or collaps of platform.      On or about 13 March 2012 at jobsite located at 2408 Mac Davis, Lubbock TX, were more tha on section of the scaffold was not laid properlly on direction change. Exposing employees to fall hazard from shift or collaps of platform. North west end of building, third tier from the top of the scaffold.

29 CFR 1926.454(a):     The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    from section (a)(1) to (c)(2)         On or about 14 March 2012, Employer did not ensure that employees were trained by employer/competent person to recognize hazards for work being performed on scaffold five stories high.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portableladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps,walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. Crossbraces shall not be used as a means of access.    On or about 14 March 2012 Employees were not provided proper access on to the scaffold platform. Employees using cross braces as access point due to placement of ladder. 29 CFR 1926.451(g)(4)(i):     Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems was not installed before the scaffold was released for use by employees other than erection/dismantling crews.      On or about 13 March 2012 employees not conducting erecting or dismantlting were allowed to work on a scaffold that was not fully guarded by guardrail system exposing them to fall hazard from an elevated work platform.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On or about 13 March 2012 employees working on an unprotected section of the top tier of a scaffold five stories high, were not protected by railling, fall arrest system or safety net exposing them to fall hazard from an elevated work platform.
