29 CFR 1926.102(a)(1):     Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) Lower Roof - Employee was observed using a powered circular saw without eye protection exposing him to eye injury hazards, on or about 04/09/12.  b) Upper Roof - Employee was observed using a pneumatic nail gun without eye protection exposing him to eye injuries, on or about 04/09/12.

29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from fallingor flying objects, or from electrical shock and burns, shall be protected by protective helmets.    a) Lower Roof - Employee was observed without head protection working directly below another employee on an upper roof, on or about 04/09/12.    The Aguilar Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 312881964, citation number 1,item number 1 and was affirmed as a final order on 05/01/09, with respect to a workplace located at 5603 16th Street, NW, Washington, DC, 20012.

29 CFR 1926.501(b)(13):     "Residential construction." Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provisionin paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it isinfeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets therequirements of paragraph (k) of 1926.502:    a) Lower Roof - An employee was observed on a porch roof without fall protection exposing him to an approximately 9 foot 5 inch fall, on or about 04/09/12.     b) Lower Roof - An employee was observed on the roof of a house without fall protection exposing him to an approximately 20 foot fall, on or about 04/09/12.    The Aguilar Home Improvement, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 312881964, citation number 1,item number 1 and was affirmed as a final order on 05/01/09, with respect to a workplace located at 5603 16th Street, NW, Washington, DC, 20012.

29 CFR 1926.405(g)(2)(iv):     Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a) West Side of House - A orange extension cord was observed, powering a circular saw, with the sheathing pulled back from the cord head exposing the employees to a potential shock hazard, on or about 04/09/12.    b) West Side of House - A orange extension cord was observed, powering a multi tap, with the sheathing pulled back from the cord head exposing the employees to a potential shock hazard, on or about 04/09/12.    c) West Side of House - A black cord was observed, powering an air compressor, with the sheathing pulled back from the cord head exposing the employees to a potential shock hazard, on or about 04/09/12.
