Location: Jobsite    a) The Milwaukee Sawzal plugged into a 120 volt, 20 amp four outlet junction box was not protected by a ground circuit interupter or an assured equipment grounding conductor program.    b) The Makita 7 1/4 inch circular saw plugged into a 120 volt, 20 amp four outlet junction box was not protected by a ground fault circuit interupter or an assurred equipment grounding conductor program.    c) The Makita JR3070CT Sawzaw plugged into a 120 volt, 20 amp four outlet junction box was not protected by a ground fault circuit interupter or an assured equipment grounding conductor program.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and at least one stair rail along each unprotected side or edge:    Location: Jobsite between the basement and first floor    The 36 inch wide, eleven riser stairway was not equipped with stair rails along both left and right open, unprotected sides of the stairway.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provisionin paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets therequirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fallprotection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complieswith 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.    Location: Jobsite    a) The 10 in 12 pitched residential roof with a 7 foot 9 inch fall distance to the street side and an 11 foot 2 inch fall to the backside of the structure was not equipped with a fall protection system.    b) The 31 foot 8 inch long opened sided floor on the backside of the structure was not equipped with a fall protection system.    KIK Builders LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 109177725, citation number 01, item number 002, issued on August 3, 2010 with respect to a workplace located at 14 Lucius Court in Milford, CT and became a final order on August 19, 2010.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    Location: Jobsite    Each employee exposed to falls from elevation were not provided with training program to instruct each employee in the recognition of fall hazards and the procedures to minimize or eliminate those hazards.    KIK Builders LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(a)(1)which was contained in OSHA inspection number 109177725, citation number 01, item number 003a, issued on August 3, 2010 with respect to a workplace located at 14 Lucius Court in Milford, CT and became a final order on August 19, 2010.
