B: On or about 04/20/12 during overhand bricklaying activities, the employer did not ensure an employee that was working on a fabricated frame scaffold was protected from a window opening that was 15 feet above the ground. L.M.R. CONSTRUCTION CO. INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD CFR 1926.451(G)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313941478, CITATION NUMBER 01, ITEM NUMBER 1B AND WAS AFFIRMED AS A FINAL ORDER ON 12/29/09, WITH RESPECT TO A WORKPLACE LOCATED AT 3145 W. 46TH ST.
