29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    A).	On or about 04/18/12, at the site of 177-41 Edgerton Road. Jamaica Estates, NY    An employee working on the lower level of the scaffold was in danger of head injury from falling objects such as, but not limited to tools and materials from other employee who on the third section of the scaffold. Protective helmets were not worn.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked:    A.	On or about 04/18/12, scaffolds at 177-41 Edgerton Road. Jamaica Estates, NY    Employees working on north side of the fabricated frame scaffolds were exposed to the hazard of falling approximately 18 ft to the ground. The working platforms were installed with only one or three scaffold planks.     ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    A).	On or about 04/18/12, at 177-41 Edgerton Road. Jamaica Estates, NY    Employees were ascending/descending a scaffold by climbing the frame and were in danger of falling approximately 18 ft to the ground. An access ladder or equivalent safe devices were not provided.      ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    A).	On or about 04/18/12, scaffolds iat 177-41 Edgerton Road. Jamaica Estates, NY    Employees applying stucco around the windows/house on the scaffold north side of the house were exposed to the hazard of falling approximately 18 ft. and 12 ft. to the ground, respectively. Guardrails or personal fall arrest systems were not provided.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
