29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. (a) First floor of Health Science Lab, University of SO. MS Gulfport Campus, Long Beach MS - On or about April 12, 2012 an extension cord with two male ends and a pigtail with multiple outlets did not have grounding prongs. The grounding prongs were missing.

29 CFR 1926.451(f)(4): Any part of a scaffold damaged or weakened such that its strength is less than that required by paragraph (a) of this section shall be immediately repaired or replaced, braced to meet those provisions, or removed from service until repaired. (a) Health/Science Lab, University of SO. MS Gulfport Campus, Long Beach - On or about April 12, 2012 the fabricated decking used for scaffold platforms had holes on each tier of the scaffold and some of the decking had cracks on the underside.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. (a) Health Science Lab, University of SO. MS Gulfport Campus, Long Beach MS - On or about April 12, 2012 the scaffold used for plastering did not have end rails on the ladder access end of the scaffold.
