29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. On or about April 5, 2012, and times prior thereto, Combo Warehouse roof, employee(s) are required to walk over gas, drain and electrical lines to access the condenser. The lines were located in the walkway and presented a tripping hazard.

29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: On or about April 5, 2012, and times prior thereto, Combo Warehouse roof, employee(s) performing preventive maintenance work on the anhydrous ammonia system must walk close to the edge of the roof with no fall protection. Employee(s) perform preventive maintenance on the fans and must access M16 valve. Employee(s) exposed to a fall hazard greater than 4 feet.
