Each employee on a walking/working surface (horizontal and verticalsurface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrailsystems, safety net systems, or personal fall arrest systems. a. On or about April 24, 2012 - at the above addressed jobsite, an employee performing carpentry operations from the roof was exposed to fall hazards, of more than twenty five (25) feet, created by the presence of unprotected sides.

29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units. a. On or about April 24, 2012 - at the above addressed jobsite, employees were performing masonry operations from a tubular welded frame scaffold, five (5) feet above the ground, that was erected on unstable objects, such as scrap lumber and masonry brick, thereby exposing employees to falls.
