29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder���s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    At the jobsite located at 212 E Sycamore, Thayer, IL, employees were exposed to a fall hazard of approximately 11.5 feet to the ground below while accessing the roof because the ladder used for access did not extend at least 3 feet above the upper landing surface.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    At the jobsite located at 212 E Sycamore, Thayer, IL, employees were exposed to a fall hazard of approximately 11.5 feet to the ground below while performing residential roofing operations on a roof with a pitch of approximately 8/12.  The employees were not provided with fall protection.

At the jobsite located at 212 E Sycamore, Thayer, IL, employees performing residential roofing activities were exposed to fall hazards and the employer failed to ensure that frequent and regular inspections of the jobsite were made to by a competent person to identify and eliminate the hazards.

The employer had not provided a training program to train employees on fall hazards and fall protection.
