29 CFR 1910.133(a)(1):     The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Well site:  On or about April 18, 2012, an employee providing rental tool services during the repair of an oil/gas well was not using protective eyewear.  This exposed the employee to the hazard of being struck in the eye by work generated debris or pressurized liquids.

29 CFR 1910.135(a)(1):     The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    Well site:  On or about April 18, 2012, an employee providing rental tool service during the repair of an oil/gas well was not wearing a hard hat.  This exposed the employee to the hazard of being struck on the head by falling objects from the service rig.

The pump stroke rods guard was missing exposing the employee to the hazards of being struck by the rod components and or being caught in pinch points.
