29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.) Egelhof Controls Corp. located at 6001 Brent Road, Toledo, OH 43611; On or about April 25, 2012, the employer did not assure employees were protected from the elevator portion of Assembly Line 427. On November 21, 2011, an employees hand was crushed in the final line elevator belt system when trying to unjam a tray.	    b.) Egelhof Controls Corp. located at 6001 Brent Road, Toledo, OH 43611; On or about April 25, 2012, the employer did not assure employees were protected from the grippers on Station 5 of Assembly Line 427. On March 27, 2012, an employees hand was crushed between the grippers when trying to replace a cover on a part.
