29 CFR 1910.212(a)(3)(ii):     Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a)  At the worksite and on April 15, 2012, where a horizontal band saw, manufactured by Hyd-Mech Series 20, Serial # unknown, exposed employees to amputation hazards by not having guards that were designed or constructed in a manner that prevents employees from having any part of their body in the danger zone during operation.    (b)   At the worksite and on April 15, 2012, where an "Ironhorse" machine, manufactured by Sonder Messern FG 628, Serial # unknown, exposed employees to amputation hazards by not having guards that were designed or constructed in a manner that prevents employees from having any part of their body in the danger zone during operation.    (c)  At the worksite and on April 15, 2012, where a Press Brake, manufactured by Chicago, Dreis & Krump MFG. Co., Model #200-F-10, Serial #  H1229, exposed employees to amputation hazards by not having guards that were designed or constructed in a manner that prevents employees from having any part of their body in the danger zone during operation.
