29 CFR 1926.96: Safety-toe footwear for employees shall meet the requirements and specifications in American National Standard for Men's Safety-Toe Footwear, Z41.1-1967.    A.	 On or about 4/19/12, on the ground floor of 82-25 Queens Blvd. Elmhurst, NY    Employees working at the site were exposed to foot injuries. The employees wore the pair of causal shoes.    ABATEMENT VERIFICATION IS REQUIRED.

On or about 4/19/12, on the ground floor of 82-25 Queens Blvd. Elmhurst, NY    Employee was cutting metal pipe with a DeWalt cut-off machine without a guard.    ABATEMENT VERIFICATION IS NOT REQUIRED.

On or about 04/19/12, on the ground level of 82-25 Queens Blvd. Elmhurst, NY     A mobile scaffold (2) wheels were not locked when an employee installing plumbing & heating system on the platform of the scaffold.
