29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:       On or about April 25, 2012, and at times prior thereto, employees cleaning and making adjustments to machines in the tortilla factory were not provided a program, procedures and specific training to lock out equipment and all unexpected energy sources.  Employees were exposed to amputation, struck by and crushed by hazards.

Employees were exposed to amputation and caught in hazards in the following:        a) In the tortilla factory, employees operated the Torcal tortilla machine and the pulleys were not guarded.        b) In the tortilla factory, employees operated the Torcal cooler conveyer and the pulleys were not guarded.

Employees were exposed to amputation and in-going nip point hazards in the following:      1) In the tortilla factory, employees operated the Torcal tortilla machine and the pulley belt was not guarded.      2) In the tortilla factory, employees operated the Torcal cooler conveyer and the pulley belt was not guarded.
