South Boundary St, Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while applying new roofing to a low sloped roof (4:12) on a commercial type structure. Employees were exposed to potential falls of approximately 20.4 feet.

South Boundary St, Perrysburg, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems. Employees were exposed to falls of approximately 20.4 feet to the paved ground below, which could result in permanent disability or death.

South Boundary St., Perrysburg, Ohio, the employer did not ensure that the side rails of an extension ladder were extended at least three feet above the landing. Employees were exposed to falls while using the ladder to access the working level to perform roofing activities. The ladder was extended approximately 1.5 feet above the roof line.
