29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    Worksite:  The employee who was engaged in residential construction activities and exposed to an approximately 19 foot fall hazard was not protected from fall hazards by the use of guardrails, safety nets or a personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Worksite:  The employee who was exposed to fall hazards of approximately 19 feet above the ground while working on the 3rd floor was not trained in fall hazards and the procedures to be followed to minimize such hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Worksite:  The extension ladder that was used to gain access to the third floor work area at a height of approximately 19 feet above the ground did not have its side rails extended 3 feet above the landing surface.
