29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.    a.  On or about February 28, 2013, the Agency (OSHA) had not received abatement documentation for Citation 1 Item 2 for Inspection number 401783 - Machine Guarding for the VM24 Millitronics.    b.  On or about February 28, 2013, the Agency (OSHA) had not received abatement documentation for Citation 1 Item 3 for Inspection number 401783 - Machine Guarding for the MM18 Millitronics.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The employer failed to properly guard this machine to prevent employees exposure to lacerations, cuts and amputations. 29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment    The employer failed to cover unused openings on the electrical panel near the exit door adjacent to VM24 and VM17 machining centers. Employees were exposed to shocks, burns and electrocution while accessing the panel and accidentally inserting their fingers in the openings. 29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    Production Area. Dust and metal chips accumulations were around the machines and workstations. This includes areas that had not been used for 3 or 4 days. Normal production operations and cleaning operations may expose employees to sharp metal pieces and chips.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
