The personnel platform was not provided with attachment means and not securely attached to the lifting carriage or forks.  The employee were exposed to hazards of falling, dropped on and/or struck by.

On or about April 19, 2012, and at times prior thereto, in the shop areas employees were allowed working from a personnel platform mounted onto a forklift truck and raised to a height more than ten feet high.
