29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: a) G. B. Construction, Inc., Bowling Green, Ohio: On or about April 25, 2012, at address 303 S. Main St., Bowling Green, Ohio, the employer failed to ensure that an employee did not stand or sit on the guardrails of a Grove manufactured, model MZ66DXT aerial lift. The employee was exposed to a fall of approximately 20 feet while standing on the mid-rail and sitting on the top of the guardrail of the previously stated lift.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) G. B. Construction, Inc., Bowling Green, Ohio: On or about April 25, 2012, at address 303 S. Main St., Bowling Green, Ohio, the employer failed to ensure that each employee working from a Grove manufactured, model MZ66DXT aerial lift was adequately protected from falls. Neither of the two employees were tied off to the basket of the previously stated aerial lift while the basket was elevated. The employees were exposed to a potential fall of approximately 20 feet to the ground below.
