29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:        On or about April 25, 2012  employees installing sheet metal decking at a construction site were using a platform that was not designed by a qualified person or constructed in accordance ANSI B56.6 standards.  The work platform was not equipped with guardrails , or a means to securely mount or attach the platform. The employees were exposed to an unprotected fall of up to 18 feet to the ground.

29 CFR 1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:        On or about April 25, 2012, employees installing sheetmetal decking at a construction site were using a platform that was elevated by a LULL 644D-34 Highlander Telehandler forklift were exposed to a fall hazard of more than 18 feet. The platform in use was not an approved work platform by the manufacturer for capacity, construction, use, and fall protection. The platform dimensions exceeded the width of the LULL 644D-34 Highlander Telehandler forklift more than 10" on each side.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:        On or about April 25, 2012, at a commercial construction roofing project, employees were installing sheetmetal decking from a platform that was elevated by a LULL 644D-34 Highlander Telehandler forklift that was not securely attached to the forklift. The employees were exposed to a fall hazard of more than 18 feet.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.        On or about April 25, 2012, at a commercial construction roofing project, employees were installing sheetmetal decking from a platform that was elevated by a LULL 644D-34 Highlander Telehandler forklift without guardrails or personal fall arrest system.  The employees were exposed to a fall hazard of more than 18 feet.

On or about April 25, 2012, at a commercial construction roofing project, employees were installing sheetmetal decking from a platform that was elevated by a LULL 644D-34 Highlander Telehandler forklift that was left unattended in a raised position. The employees were exposed to a fall hazard of more than 18 feet.
