29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a. On or about April 4, 2012 - at the above addressed jobsite, employees installing an eight (8) inch water main inside a seven (7) foot deep trench were not provided with an adequate protective system, thereby exposing the employee to cave in hazards. THE RELIABLE CONTRACTING AND EQUIPMENT CO. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313934945, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 14, 2010 WITH RESPECT TO A WORKPLACE LOCATED AT 3918 W. CHICAGO AVE., CHICAGO, IL 60651. THE RELIABLE CONTRACTING AND EQUIPMENT CO. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313934721, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 14, 2010 WITH RESPECT TO A WORKPLACE LOCATED AT 1240 W.
