29 CFR 1926.451(g)(4): Guardrail systems installed to meet the requirements of this section did not comply with the provisions listed in subparagraphs (i) through (xv) of this paragraph. a) I Con Building & Supply Inc. On or about April 25, 2012 employees were working from a scaffold platform erected 22 inches below the top plate exposing employees to falls from 33 feet above ground level due to the top plate being of insufficient height to act as a proper guardrail.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: a) I Con Building & Supply Inc. On or about April 25, 2012 employees were working on a supported scaffold consisting of a single wooden plank from 3 1/2 to 5 1/2 inches wide. The wood planks were erected 6 feet 3 inches above floor level inside the exterior wall of the fourth floor that was 33 feet above ground level.
