(a) Jobsite: 1505 Buttermore Boulevard, Connellsville, PA 15425; On or before April 26, 2012, employee(s) were working unprotected and exposed to fall hazards while using a ladder jack scaffold plank approximately 17 feet 6 inches from ground level that extended approximately 4 feet beyond its support.

29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    (a) Jobsite: 1505 Buttermore Boulevard, Connellsville, PA 15425; On or before April 26, 2012, employee(s) were working unprotected and exposed to fall hazards while using a ladder jack scaffold approximately 17 feet 6 inches from ground level. Employee(s) were using the scaffold to collect shingles from behind the 2x6.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    (a) Jobsite: 1505 Buttermore Boulevard, Connellsville, PA 15425; On or before April 26, 2012, employees were working unprotected and exposed to fall hazards of approximately 18 feet 6 inches to ground level while on a 12/12 pitched roof. Employees were installing new shingles on the roof.
