29 CFR 1926.451(g)(1)(i):   Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system;  On or about 4/4/12 employees working on Ellington Travel roofing project were exposed to approximately 12' falls.  An employee was engaged in roofing activities from a ladder jack scaffold without wearing a personal fall arrest system.

29 CFR 1926.501(b)(11):   "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) ormore above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrestsystems.  On or about 4/4/12 employees working on the Ellington Travel roofing project were exposed to approximately 15' falls.  An employee was engaged in roofing activities without fall protection.
