29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Northwest corner of the site, adjacent to the site entrance; On or about April 25, 2012 and at times prior thereto, employees installing a concrete manhole structure and related PVC wastewater piping in an excavation up to 13 feet deep with nearly vertical excavated walls were utiliziig the excavated slope for access in/out of the excavation. Sections of the slope exceed 45 degrees. Note: Abatement certification or documentation is NOT required for this item.
