Only one guardrail was provided at approximately 36 inches above the walking working surface. Employees used a ladder set up in the area to access the floor from a lower level. Employees were exposed to falling 1-2 stories to lower levels.    b) Wall openings were not adequately guarded and personal fall arrest systems were not in use. Employees were exposed to the hazard of falling 1 - 2 stories.    c) A ramp used by employees to access the house from the front yard did not have guardrails. Parts of the ramp were approximately 10 ft above lower levels. Employees were exposed to the hazard of falling to lower levels.
