29 CFR 1910.146(c)(4):     When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:    a) At the site, in the dipping tank areas, where dipping tanks contain corrosives, an employee is required to clean the inside of tanks twice a year. No written confined space entry program has been developed, exposing the worker to potential inhalation, dermatological, eye, or entry hazards.    b) At the site, in the area between the Machine Shop and the Warehouse for Rod, a CMC Clearing Crankless forging press is used which is placarded as a confined space.

29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    In the washing dip tank area, where corrosive material is used in dip tanks to clean manufactured parts, there is no eyewash station in the immediate area. The only available eyewash is located in the next room, on the other side of an adjacent machine, exposing the worker to eye damage while trying to reach the eye wash.
