29 CFR 1910.333(a)(2):  Safety-related work practices were not used to protect employees who were exposed to electrical hazards.   Such work practices were not in place and were not protecting employees against contact with energized circuits or parts.    a) Injection Molding Room - The employer failed to ensure safety-related work practices were used to protect employees who were exposed to electrical hazards while working on equipment, such as the Hanyoung #HY-V908 Limit Switch located on Vistar Injection Molding machine #5; SN #200078.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.333(b)(2)(i):  The employer did not maintain a written copy of the procedures outlined in paragraph(b)(2) and did not make it available for inspection by employees.    a) The employer did not maintain a copy of the written procedures on electrical safety hazards.   Procedures were not made available and as a result the employees were exposed to electrical hazards while working on or near energized parts and equipment, such as the Hanyoung #HY-V908 Limit Switch located on Vistar Injection Molding machine #5; SN #200078.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
