29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Jobsite, Detroit Switch Building, 1025-33 Beaver Avenue, Pittsburgh, PA: On or before 4/25/12, employees were engaged in roofing activities within 6 feet of the edge of the roof.  Employees were within 6 feet of the edge roof of the roof when moving equipment and materials  which exposed them to a 62 foot fall hazard.

29 CFR 1926.502(f)(1)(iii):     Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:    a) Jobsite, Detroit Switch Building located at 1025-33 Beaver Avenue, Pittsburgh, PA: On or before 4/25/2012, employees entered the roof area from an interior building stairway and a point of access was not provided to the work area designated by the safety warning lines. This condition required the employees to travel within 2 feet of the edge of the low-slope roof and exposed them to an approximate 62 foot fall hazard.

The employees were using Verisgard substrate adhesive, a flammable liquid, to install the roofing system, and approximately 100 gallons of the adhesive was stored at the jobsite.
