29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a) At the worksite, the employer did not ensure that hard hat protection was utilized by employees working below employees working above their head, thereby exposing ground level working employees to a potential struck-by hazard.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) At the worksite, the employer did not ensure that employees were protected from fall hazards associated with working from a pump jack scaffold platform at heights greater than 10 feet.
