29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Entire Facility.  The employer did not have an established lockout/tagout program for employees who performed servicing and maintenance on machines and equipment such as but not limited to machining centers, lathes, saws, etc.      Abatement Note:  A lockout/tagout program should include the following key elements:    - Energy source evaluation  - Development, documentation and utilization of machine-specific procedures  - Procedures for group lockout (when more than one employee is servicing the same equipment at the same time)  - Training for "authorized", "affected" and "other" employees  - Periodic inspections (at least annual)  - Sharing of lockout/tagout program information with outside contractors    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    The guard doors on the following CNC machines were not secured during operation or equipped with a safety switch that would automatically stop the machine if the guard door was opened during the machining cycle.    a)  Mazak CNC Machining Center #210 - If door is opened during operation, the machine will continue running.    b)  Mazak CNC Machining Center #222 - If door is opened during operation, machine will finish cutting cycle and then turret stops.  Operator must hit reset button to stop spindle from turning.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a)  Press Brake (Manufacturer: Stanco Welder & Machine Co.).  Employee(s) operated the brake press with a foot pedal while hand-feeding small parts with their hands within 3-4 inches of the point of operation.  The press brake did not have any point of operation guarding.    b) Shear (Manufacturer: George A. Ohl & Son).  The openings in the point of operation guard on the shear were too large to prevent employees' fingers from entering the point of operation when cutting corrugated cardboard.    d)  Wotan Machining Centers.  There was no point of operation guarding to protect the operator from getting his hands or other body part into the point of operation while running.      e) Iron Worker Machine.  The point of operation was not guarded by a guard or restricting the size of the opening in the point of operation to 1/4".     f)  Dewalt Metal Sutting Radial Saw #706.  The saw did not have a self-adjusting guard covering the exposed lower portion of the blade.    Abatement documentation is required for instances a), c), e) and f) in accordance with the requirements of 29 CFR 1903.19(d).    There is no abatement certification or documentation required for instances b) and d), which were Corrected During Inspection.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the stock and remained in contact with the stock being cut to give maximum protection possible for the operation being performed.    Shipping Dept.  The Dewalt #740 Radial Saw used for cutting wood for shipping pallets did not have a self-adjusting guard covering the exposed lower portion of the blade.    There is no abatement certification or documentation required for this item.
