29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) East to west ends of excavated area; On or about April 24, 2012 and at times prior thereto, employees installing PVC piping associated with the construction of a water storage tank were not provided with an immediate means of egress and/or were required to travel approximately 50 feet to exit the excavation. Note: Abatement certification Is required for this item.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: a) East end of excavation, south section of waterline bypass; On or about April 25, 2012, and at times prior thereto, three employees were installing PVC piping in areas of the excavation at 6 feet in depth with nearly vertical walls and no means of protection provided. b) East end of excavation, north section of the excavation adjacent to the existing manhole structure; On or about April 25, 2012, and at times prior thereto, three employees were installing PVC piping in areas of the excavation at 10 feet in depth with nearly vertical walls and no means of protection provided. Note: Abatement certification IS required for this item.
