The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:                     a) On or about 04/27/12 CSHO observed employees accessing the roof using a Werner 24 foot aluminum extension ladder that did not extend three feet above the landing.
