29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) On or about 04/27/12 CSHO observed four men working doing rip and tear of old roofing materials with no fall protection system in place.

29 CFR 1926.1053(b)(1):     Portable ladder was used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. The portable ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:                     a) On or about 04/27/12 CSHO observed employees accessing the roof using a Werner 24 foot aluminum extension ladder that did not extend three feet above the landing.
