29 CFR 1926.451(e)(5)(i): Ramps and walkways 6 feet (1.8 m) or more above lower levels shall have guardrail systems whichcomply with subpart M of this part -- Fall Protection; Worksite: Walkways were placed between two scaffolds without a guard rail system. One of the walkways had rutted planks with holes and weakens sections exposing employees to the fall hazards from 18 feet to 6.5 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrailsystems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. Worksite: An employee was observed working about 30 feet on top of a fabricated metal scaffold platform and was not provided with side rails to prevent him from falling from the sides.

Worksite: The employer did not provide a mean to access or egress the top working levels of a 30 feet prefabricated metal scaffold.
