29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation had been performed; the dates of the hazard assessment; and, which identifies the document as a certification of hazard assessment:     In the facility, on or about June 12, 2012, the employer did not provide written certification that a workplace hazard assessment had been completed that identifies the workplace evaluated; the name of the person conducting the assessment; the date of the assessment; and identification of the document certifying completion of the hazard assessment.

29 CFR 1910.1026(d)(1): The employer did not determine the 8-hour time-weighted average (TWA) exposure for each employee exposed to chromium (VI) in a workplace or work operation covered by this section:     In the facility, on or about June 12, 2012, where employees are performing MIG and TIG welding operations on stainless steel, the employer did not determine the 8-hour TWA exposure for each employee exposed to Chromium (VI).
