Truck Sign Room - On or about April 24, 2012, employees were required to operate the following rivet machines without guards: 1. National Rivet & Mfg. Co. 2. Chicago Rivet & Machine Co. Employees were thereby exposed to pinch point hazards from moving parts.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. a. Entire facility - On or about April 24, 2012, the employer had not adequately certified periodic inspections of the established energy control procedures in place for employees who perform servicing and maintenance of equipment. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
