29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    a) On or about April 27, 2012, at the jobsite located at 208 East 61st Street, Chicago, IL - an employee performing tuckpointing activities from a tubular welded frame scaffold that was not fully planked or decked was exposed to fall hazards of approximately thirty (30) feet.

29 CFR 1926.451(g)(3)(i): When vertical lifelines are used, they shall be fastened to a fixed safe point of anchorage, shall be independent of the scaffold, and shall be protected from sharp edges and abrasion. Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights:    a) On or about April 27, 2012, at the jobsite located at 208 East 61st Street, Chicago, IL - an employee was performing tuckpointing activities from a tubular welded frame scaffold was using a vertical life line that was attached to the scaffold, thereby exposing the employee to fall hazards of approximately thirty (30) feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level:    a) On or about April 27, 2012, at the jobsite located at 208 East 61st Street, Chicago, IL - Employees working from a tubular welded frame scaffold performing tuckpointing activities on a chimney were using rigging that allow them more than a six (6) feet free fall, thereby exposing the employees to fall hazards of approximately thirty (30) feet.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder:    a) On or about April 27, 2012, at the jobsite located at 208 East 61st Street, Chicago, IL - Employees performing tuckpoiniting activities were using a ladder that did not extend three (3) feet above the upper landing surface, thereby exposing employees to fall hazards of approximately thirty five (35) feet.
