29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    (a) Employees were conducting roofing work without any fall protection system in place. The employees were exposed to a fall hazard of approximately 9 feet.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee who might be exposed to fall hazards:    (a) The employer did not ensure that employees were using fall protection when conducting work on the roof while being exposed to a fall hazards. The employer did not ensure that the employees were trained to recognize and protect themselves from fall hazards.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) The ladder that employees used to access the roof did not extend at least 3 feet above the access level. This exposed employees to a possible fall hazard of approximately 9 feet to soft ground below.
