29 CFR 1926.453(b)(2)(v):   A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:  a) Southwest corner of structure: A worker used a JLG 400S telescoping boom lift to access the roof of a structure that was approximately 30 feet above the ground. A harness and lanyard was not used exposing the worker to a fall, on or about April 26, 2012.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.454(a):   The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Pittston, PA worksite: Employees exposed to fall hazards while working from a JLG 400s telescoping boom lift were not trained in the hazards of falling, on or about April 26, 2012.  ABATEMENT CERTIFICATION REQUIRED
