29 CFR 1926.451(b)(1):     Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights:     (a) Upper Side of the Building: The fabricated frame scaffold which measured approximately 5 feet wide by 7 feet long was not fully planked at the working level approximately 17 feet from the ground due to the fact the employee was working from 2 scaffold planks which measured approximately 19 inches wide as observed on 4-18-12.

29 CFR 1926.451(f)(7):     Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    a. Upper Side of Building:The fabricated framed scaffold which was approximately 5 feet by 7 feet with the working platform approximately 17 feet off the ground was not fully decked and was not equipped with a guard rail system nor was it erected under the supervision and direction of a competent person as determined on 4-18-12.    Abatement Certification Is Required For This Item

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a. Upper Side of the Building: An employee was exposed to a fall of approximately 17 feet to the ground below due to there was no fall protection on the fabricated frame scaffold where the employee was working from two scaffold planks as observed on 4-18-12.
