29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: At the job site located at 422 County Stone Drive, Lake St. Louis, MO, 63367, employees were exposed to a fall hazard as they were not protected by any type of fall protection system while they actively performed roofing work. The eave to ground height measured approximately 22 feet at the front of the residence and approximately 11 feet at the back doorway.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Three employees gained roof access and egress by utilizing portable ladders which did not extend at least 3 feet above the upper landing (roof). The side rails measured approximately 18 inches above the landing at the front ladder access and the side rails measured approximately 6 inches above the landing at the back doorway access. The fall hazards at both ladder locations measured approximately 11 feet.
