Hazardous chemicals in the workplace included, but are not limited to the following:    (1)  Bronze ingots;  (2)  Ransom & Randolph Ranco-Sil fused silica;  (3)  Bareco Products Victory Brown Wax;  (4)  Ransom & Randolph Primcote binder;  (5)  DuPont Starblast;  (6)  Propane; and   (7)  Oxygen.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area:    Lee West Enterprises, dba Lee West Studios, at 2855 West 8th Avenue, Denver, CO:  On and preceding 5/1/12, Lee West Enterprises did not provide effective information and training as specified in 29 CFR 1910.1200(h) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area. 29 CFR 1910.305(g)(1)(iv)(A):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    Lee West Enterprises, dba Lee West Studios, at 2855 West 8th Avenue, Denver, CO:  On and preceding 5/1/12, Lee West Enterprises did not ensure that flexible cords and/or cables were not used as a substitute for the fixed wiring of a structure.   Fixed overhead fluorescent lighting was used to light the room where ceramic shells were fabricated.  Flexible cords were used in place of fixed wiring.  This condition exposed one employee to an electrical hazard.    Lee West Enterprises, dba Lee West Studios, at 2855 West 8th Avenue, Denver, CO:  On and preceding 5/1/12, Lee West Enterprises did not ensure that flexible cords and/or cables were not used as a substitute for the fixed wiring of a structure.   Fixed room fans were used to dry ceramic shells in the room where ceramic shells were fabricated.  Flexible cords were used in place of fixed wiring.  This condition exposed one employee to an electrical hazard.    Lee West Enterprises, dba Lee West Studios, at 2855 West 8th Avenue, Denver, CO:  On and preceding 5/1/12, Lee West Enterprises did not ensure that flexible cords and/or cables were not used as a substitute for the fixed wiring of a structure.   Two abrasive blasting cabinets were permanently fixed in the shop.  Flexible cords were used in place of fixed wiring.  This condition exposed one employee to an electrical hazard.

Employees performed tasks at a nonferrous foundry.  Employees during the course of their duties were exposed to flying particulates, noise, and respiratory hazards.  The employer had not performed a written certification of the workplace hazard assessment.
