29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 4/26/12, Extrusion AFE: Employees working at the 6500 Press were not utilizing lock out procedures as written. When sleeve changes are performed, the operator locks out some controls for the person changing the sleeves. Abatement certification is required for this item.
