29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Facility Wide: Employees perform maintenance and service on machinery such as, but not limited to the Polar Mohr 92EM cutter and requires the control of hazardous energy by locking out the equipment. The employer has not conducted a periodic inspection of the energy control procedures, as determined on or about 04/17/2012. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Facility Wide: Employees perform maintenance and service on machinery such as, but not limited to the Polar Mohr 92EM cutter and requires the control of hazardous energy by locking out the equipment. The employee failed to utilize the hazardous control procedures. As an example the employee removed the lock from electrical shutoff and reenergized the machine by turning the electrical shutoff on and then proceded to make manual adjustments to the flywheell, as observed on or about 04/17/2012. ABATEMENT CERTIFICATION REQUIRED
