29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about May 01, 2012, employees who were installing roofing material were not provided adequate fall protection. The employees' lanyards had too much slack rendering the fall protection system ineffective. The employees were exposed to falls of 14 feet to the ground below.
