29 CFR 1910.1200(h)(1):  Employer failed to provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about was introduced into their work area.     (a)  Triad Service Solutions, 4567 East 9th Ave., Denver, CO 80220:  On and before April 27, 2012, the exposing employer failed to conduct effective training on the hazards associated with the charging lead batteries for the floor buffers.  Lack of effective training resulted in the employees introducing the water into the batteries, leaving the batteries caps off, then departing the work area allowing the batteries to boil over releasing poisonous gases into the atmosphere.  Condition exposed employees and hospital employees and patients to dangerous gases.
