McCormick Structural Systems, Inc., 1023 French Street, Erie, PA 16501, construction trailer, on or about April 30, 2012 and times prior thereto: A electrical panel box with live circuits was located in a construction trailer. The electrical panel box was not equipped with a dead front. B.) McCormick Structural Systems, Inc., 1023 French Street, Erie, PA 16501, construction trailer, on or about April 30, 2012 and times prior thereto: When the missing dead front was placed on the electrical panel box, the dead front was missing blanks where breakers were not present.

McCormick Structural Systems, Inc., 1023 French Street, Erie, PA 16501, west side of the site, on or about April 30, 2012 and times prior thereto: A employee was standing on the mid rail of a JLG 2032E2 scissors lift to tie in structural steel. The platform of the sissors lift was at 20 feet above ground level.

A 17" gap was present between the stairs and the construction trailer.
