29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      The employer does not provide employees with a program entailing written energy control procedures, training, and periodic inspections for conducting lockout/tagout activities when performing services and/or maintenance and setup to equipment or machinery. This violation was observed on or about April 27, 2012, in the fabrication shop where employees were exposed to electrical, caught-between and struck-by hazards while performing services and/or maintenance and setup to equipment or machinery without energy control procedures.

This violation was observed on or about April 27, 2012, in the fabrication shop, where employees were exposed to caught-in and struck-by hazards when operating a metal cutting band saw that does not have the unused portion of the blade guarded.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s)      The employer does not guard the point of operation of machines whose operation exposes an employee to injury.    a) This violation was observed on or about April 27, 2012 in the fabrication shop area where employees were exposed to caught-in and caught-between hazards while operating an IMCAR 1.5 ton roller that had its safety peripheral wire removed.     b) This violation was observed on or about April 27, 2012 in the fabrication shop area where employees were exposed to caught-in and struck-by hazards while operating an Accurpress Press Brake without a point of operation guard.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.      The employer does not ensure that electrical panels are readily accessible.  This condition was observed on or about April 27, 2012, on the south wall of the fabrication area where employees were exposed to a delayed emergency response hazard where access to an electrical panel was being blocked by cylinders, a welder car, pipe jack stands, and metal parts.
