Delaware St. in Denver, CO: On and before May 9, 2012 the employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involved eye hazards wore prescription safety glasses or secondary eye protection over prescription glasses. This condition exposed employees to eye hazards during plating tasks.
