29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. a) Building number 4 - On or about 2 May, 2012 - Two employees were exposed to a fall hazard while installing metal roofing panels and not wearing conventional fall protection. Employees were exposed to a fall height of 22 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Building number four - On or about 2 May, 2012 - Employees were exposed to a fall hazard while using a ladder to gain access to the roof that was only extended above the landing 24 inches. Employees were exposed to a fall hazard of 22 feet.
